Orthopedics Billing and Coding Guide

Orthopedic billing is built around global surgical periods and bundled codes: a single procedure code often includes the pre-operative visit, the surgery itself, and a defined window of follow-up care, and most orthopedic denials trace back to billing something that was already bundled into that package.

Why This Specialty Is Billing-Complex

Fracture care, joint injections, and joint replacement codes each carry a global period (0, 10, or 90 days) during which related E/M visits and follow-up procedures are not separately payable unless a modifier documents that the work was unrelated or staged. Add laterality (left/right/bilateral) and multiple joints treated in one visit, and the modifier logic gets complicated fast.

Common CPT, ICD-10 & HCPCS Codes

  • CPT 20610 — arthrocentesis/injection, major joint or bursa
  • CPT 29881 — knee arthroscopy with meniscectomy
  • CPT 27447 — total knee arthroplasty
  • CPT 25600-25605 — treatment of distal radius fracture
  • CPT 73030 / 73560 — shoulder and knee X-ray
  • Modifiers 58, 78, 79 — staged, related, and unrelated procedures during a global period
  • Modifiers RT/LT/50 — laterality and bilateral procedures
  • ICD-10 M17.11, M25.561, S52.501A — osteoarthritis, joint pain, and fracture examples

Documentation Standards

Operative notes should state the global period assigned to the procedure and, for any follow-up work billed during that window, explicitly document why the additional service was unrelated to the original surgery or was a planned stage of treatment. Laterality must be stated in every note, not inferred from the schedule, since a missing RT/LT/50 modifier is one of the most common reasons a clean-looking orthopedic claim still denies.

Common Denial Reasons & How to Prevent Them

  • Global period bundling denials for E/M visits or minor procedures performed during the post-op window without modifier 58/78/79
  • Modifier 59/X{EPSU} misuse when unbundling codes that are correctly bundled under NCCI edits
  • Missing or incorrect laterality modifiers causing duplicate-claim or bundling denials
  • Medical necessity denials for joint injections without documented failed conservative treatment

Global Periods and the Modifiers That Untangle Them

Every surgical CPT code carries a global period — 0, 10, or 90 days — defining how long routine post-operative care is bundled into the original procedure’s payment rather than separately billable. Four modifiers govern most of the disputes that arise inside that window: modifier 24 (a significant, separately identifiable E/M service on the same day as a minor procedure), modifier 58 (a staged or related procedure planned at the time of the original surgery — common for staged hardware removal), modifier 78 (an unplanned return to the OR for a complication related to the original surgery, such as urgent hardware revision), and modifier 79 (an unrelated procedure by the same physician during another procedure’s global window, such as a different body part). Choosing the wrong one of these four is one of the most common orthopedic denial patterns.

Bundled Codes and NCCI Edits

National Correct Coding Initiative (NCCI) edits define which codes are considered bundled into others when billed together. A classic orthopedic example: arthroscopic procedures often bundle diagnostic arthroscopy into a more extensive therapeutic arthroscopic procedure performed in the same session, so billing the diagnostic scope separately when it was simply the first step of the therapeutic procedure is a standard denial pattern. Casting and strapping supplies and application are also frequently bundled into fracture-care codes — billing them separately on top of a global fracture-care code requires meeting specific circumstances (such as a different physician applying the cast than the one performing definitive fracture care), not just documenting that a cast was applied.
