Dermatology billing hinges on one recurring distinction: whether a procedure is medically necessary or cosmetic. Payers only cover the former, so the documentation has to make that line explicit for every biopsy, excision, and lesion destruction billed.
Why This Specialty Is Billing-Complex
Many dermatology procedures (mole removal, skin tag removal, scar revision) can be performed for either a medical reason (suspicious lesion, symptomatic irritation) or a cosmetic one, and the CPT code alone does not tell the payer which applies. Pathology results, when available, are the strongest evidence of medical necessity, but not every procedure generates a pathology report.
Common CPT, ICD-10 & HCPCS Codes
- CPT 11100-11101 — biopsy of skin, single and each additional lesion
- CPT 17000-17004 — destruction of benign/premalignant lesions
- CPT 11400-11446 — excision of benign lesions
- CPT 11600-11646 — excision of malignant lesions
- ICD-10 L82.1 — seborrheic keratosis
- ICD-10 D22.x — melanocytic nevi
- ICD-10 C44.x — other malignant neoplasm of skin
Documentation Standards
The chart note should describe the specific clinical reason the lesion was addressed (bleeding, rapid growth, pain, irritation from clothing, suspicious appearance) rather than just naming the procedure. When a lesion is sent to pathology, the report should be referenced in the billing note and its findings should support the diagnosis code submitted on the claim.
Common Denial Reasons & How to Prevent Them
- Denials or patient-responsibility determinations when documentation does not clearly separate cosmetic intent from medical necessity
- Denials for lesion destruction/excision when the diagnosis code does not match a covered indication under the payer’s policy
- Bundling denials when multiple lesions are billed without the correct additional-lesion add-on codes
- Missing modifier on same-day biopsy and destruction of a different lesion
